A Home Care Package (HCP) through My Aged Care provides coordinated support and services to help you continue living safely and independently in your own home.

You can choose the provider that suits you best, while the Australian Government funds your package to ensure your care is tailored to your individual needs.

Who can receive this funding?

You may be eligible if you:

Are 65 years or older, or
Are 50 years or older for Aboriginal or Torres Strait Islander people

Eligibility is based on an aged care assessment conducted by an assessor who discusses your care needs and then determines the level of care you need.

You may also be eligible if you have:

Noticed changes in your memory or ability to manage daily tasks
Been diagnosed with a medical condition or have reduced mobility
Experienced changes in family or care support
Had a recent fall or hospital stay

How much funding is available?

Home Care Packages are provided at different levels, depending on the level of care and support you need.

Funding is paid directly to your chosen provider, who will work with you to deliver services and support tailored to your situation.
